Provide timely, effective early intervention, to reduce the need for more intensive and intrusive services. To provide outreach services in locations within the community for youth 12-18 and their families. Provide navigation to additional services as required.


JOB RESPONSIBILITIES may include but are not limited to:

  1. Must be able to work flexible hours to meet the needs of the youth/families (at least 2-3 nights per week and weekends) with the flexibility to do split shifts
  2. Must be able to work in multiple locations within the community, engage and maintain community partnerships
  3. Provides a range of brief outreach services to at-risk youth and their families including drop-in programs and individual counselling
  4. Responsible for community outreach, prevention and intervention with at-risk youth ages 12-18 to provide mental health services
  5. Participate in community committees including Brantford Crisis Table and local events



  • Must have Child & Youth Worker diploma or degree/diploma in related field
  • Minimum of three years’ experience in related field
  • Experience working with youth, families, and community partners
  • Ability to engage at-risk and/or disconnected youth and to work from a strength-based, collaborative perspective
  • Strong organizational, problem solving and decision making skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid driver’s license and daily use of a vehicle
  • Excellent verbal, written communication and computer skills
  • Current police check, including vulnerable screening
  • PMAB and ASIST certificate an asset
  • Valid First Aid and CPR (asset)
  • Second language an asset
